"Whenever the subject of effective communication is raised I think of a case that I studied in a course in high school. Avianca Flight 52 circled Kennedy Airport for almost 2 hours before crashing when it ran out of fuel. It was constantly in touch with the tower and although it mentioned being low on fuel it never declared that it was running out of fuel until its engines flared out. Details of this bizarre incident, including transcripts of all radio conversations are available on the web. (The Aviation Safety Network http://www.aviation-safety.net/cvr/cvr av052.htm reproduces the transcript and http://pw2.netcom.com/~asapilot /52.html documents the incident, including photographs and related links.
"In January 1990 Avianca Flight 52 crashed on Long Island killing almost half of the 158 persons on board. Unlike many aviation tragedies the reasons for this disaster were never in doubt. For almost 2 hours it had circled Kennedy Airport, repeatedly announcing that it was "running low on fuel." At 9:24 p. m. a scheduled landing was aborted and less than ten minutes later it simply ran out of fuel, the engines shut down, and it crashed. Throughout the two hours preceding the tragedy it was in constant contact with the air traffic controllers at Kennedy. The disaster was not linked to mechanical failure or sabotage; it was the result of ongoing but ineffective communication."
