Description:

The paper discusses how in the contemporary world of managed care, there are times when the DSM model, often requried by insurance companies and HMO paperwork, contributes to a too hasty, generalized, or even misdiagnosis. The paper points out that the point of having a robust and peer-reviewed template to assess a patient is to allow for a professional, unbiased diagnosis of the patient's individual issue so that an appropriate treatment plan may be recommended, however, some data suggests that because of the pressures of coding, misdiagnosis is more common than one would think. The paper addresses the role of research into the patient's history and the backup data on the illness itself and relates that it will allow a greater chance to provide the appropriate treatment through the appropriate channels. The paper concludes that like all tools, the DSM has the potential to be both a positive or negative force, depending on its usage and interpretation.

From the Paper:

"In order for a medical professional to have a comprehensive view of the individual client before recommending a treatment program, an assessment is needed. This involves gleaning information from a variety of sources; interviews, tests of ability, tests of interests, medical data, and other forms of informative questionning. Collateral information is also used - work history, previous records from teachers or other interviewees, and even documentation about the activities of the client. For the assessment to be useful it must be valid, that is it must be clear enough to provide evidence to support any supposition or diagnosis, as well as reliable - consistent over time and source. In most cases, using the DSM-IIIR, clinicians are provided with a useful template which allows data to be collected rationally and without prejudice (Mead, Hohenshill and Singh, 1997).
"In a clinical setting, for instance, the use of the DSM-IIIR provides a way to pre-organize patient data in a structured manner, allowing the therapist the opportunity to approach the individual case in a manner that quickly results in certain patterns and issues. In one particular case from, it was quite clear by using the DSM template that the patient in question had a history of substance and emotional abuse and to track the attempted treatments and hospitalizations (Bufford, 2008). However, in the contemporary world of managed care, there are times when the DSM model, often requried by insurance companies and HMO paperwork, contributes to a too hasty, generalized, or even misdiagnosis."
