"The identified problem is that the patient education materials that we have available consist mainly of teaching handouts and discharge instructions for heart failure patients that are generic and very general. These materials are distributed in a perfunctory manner as literature for the patient to review independently and without professional guidance or explanation of the importance of following recommendations and instructions for optimal recovery. Currently, we do not have a designated trained member of the nursing staff who is responsible for educating heart failure patients on a one-on-one basis at any time during their hospitalization or in conjunction with their discharge for the purpose of increasing patient compliance and educating patients about the crucial importance of following medical recommendations for their optimal recovery.
"This institution does follow JCAHO's "Heart Failure Core Measure Set" for discharge instructions. However, in the clinical setting our experience strongly suggests that there is a need for more focused patient education protocols to ensure that discharge instructions are understood. Patients must receive sufficient one-on-one instructions to appreciate the importance of post-discharge instructions and recommendations to support self-management capable of preventing re-hospitalization."
