This paper presents a plan for a new program called "Connections" that is designed to bring community resources together in a concerted effort to meet the basic needs of the community's struggling citizens. The paper describes the community hospitals and the Job and Family Services organization and shows how the major service delivery and support activities in the value chain differ for both organizations. "Job and Family Services is organized under the state statutes, with each county represented by its own county-level agency. The Job and Family Services of the states and of the individual counties is funded by Federal and State level grants. The purpose of these grants is to allow each individual agency to provide for the needs of the local citizens of its community. These agencies are responsible for providing a number of services for the people of the county. Each counties mission and vision are closely tied to providing these services. Recently economic conditions have produced additional hardships to those that the agency typically deals with. The following will outline a new proposed program that will help these agencies, in conjunction with local hospitals, to address new community concerns in order to be able to meet the growing needs of the individual communities that they serve."The community has several local hospitals that are dedicated to serving the needs of the local community. These community hospitals are all private organizations that are operated by a central board. This board consists of professionals, accountants, and key stakeholders within the local community. They must approve all decisions that concern the strategic plan and operations of the hospital. All five community hospitals in the area have missions that are worded differently, but that involve their ability to address the health care needs of the local community. Their strategic plans revolve around this ability."
