Therefore, it became necessary to fully document all activities of medical personnel, in both hospitals and medical practices, in order to provide durable proof that medical ethics were not being flouted. Often the hospital or medical practice did not have the capacity to type up all activities. Thus, personnel would voice-record all of their activities, and the voice recordings would then be sent out to an external company for transcribing and typing.
