Abstract This paper describes the problem of insufficient healthcare in America affecting mostly the near-poor. This is the sector of people who are not poor enough to qualify for Medicare or Medicaid programs and, the paper points out, a sector that is growing. The paper compares the U.S. healthcare system to a sieve, where many members of the population fall through the holes and then takes a look at two possible solutions to the, briefly analyzing their advantages and disadvantages.

Most of these are people who earn too little to pay premiums on individual health insurance policies or are not employed by companies that offer health insurance benefits. In spite of this, we continue to treat illegal immigrants and their dependents the same as legal residents. Over the last several decades, and despite the severe fiscal crises faced by local and state governments, political leaders continued to promote policies that grant costly benefits to people who violate immigration laws. The four major topical areas discussed are rural hospitals, rural health clinics, managed care in rural areas, and technology in rural healthcare. The paper contends that there are solutions to the problems confronting rural healthcare, but that policy changes and attitudinal changes are necessary first.

Abstract The measurement of access to healthcare in America is far from uniform. Even a cursory review of the literature reveals diverse methods, instruments and procedures for assessing access, each applied to diverse populations (e.g., poor, special needs, middle class, insured, uninsured, etc.) for diverse services (oncology, preventative care, prenatal care, etc.). This paper examines access to healthcare as related to the measurement of access and to the range of programs that have been developed to improve access. Metrics based on this perspective allow the managers to know how well their business is running, and whether its products and services conform to patient's requirements. These metrics have to be carefully designed by those who know these processes most intimately, usually includes the identification of mission-oriented processes, and support processes. Mission-oriented processes are the special functions of government offices, whereas the support processes are more repetitive, and easier to benchmark and measure using generic metrics. The patient perspective can be improved by enhancing the quality of care, and perhaps by offering additional services needed by the physicians that are not currently addressed or not stressed enough. The final perspective, financing healthcare, involves timely and accurate funding where the implementation of a corporate database would ideally centralize the majority of the processing of the data in addition to automating the processing of financial data.
