"Certainly his life on the street has probably left him malnourished, as well as suffering from drug and alcohol-related problems. At a minimum, he needs a complete physical examination, with particular attention to any exposure to hepatitis, HIV, tuberculosis, other infectious agents, including STDS, and the state of his liver. A nutritional analysis would also be helpful. Mr. Y appears to be both undernourished and bloated, which is not a good sign. He does not, however, appear to be jaundiced, which is a positive. Currently, he does not report any noticeable physical problems, except for his feelings of withdrawal and the misery that that causes for him."
