From the Paper:

"Computers have enabled businesses and organizations to minimize the cost and physical space requirements of record keeping. In primary care, accurate record keeping is particularly important for the purpose of follow-up and focused care services. Currently the Clinic under investigation does have several computerized information management systems, each customized to meet the needs of its specific department. The problem is that there is no centralized network that monitors and integrates all the separate systems."
